All state required vaccinations need to be up to date by Oct. 1, 2021, or in process with documentation that they are in process from a provider’s office or health department. Otherwise these students will be kept from attending school. Exemption forms are available at both offices if needed. For clarification, this is for all regularly required immunizations.
